Case Management
We recognize that food isn’t the only thing affecting your life.
We’re here to help break cycles of crisis and provide stability for you and your family by addressing financial insecurity.
We do this through helping you apply for public benefits like SNAP and Medicaid, which can often be an overwhelming process. We will also help you work toward economic security by connecting you to community resources in areas including: childcare, mental health support, legal help, housing, education, job support, and financial coaching.
How does it work?
When you meet with a case manager, we’ll start by filling out a strengths and needs assessment together. This will help identify your intrinsic strengths and support systems, as well as areas where you need some extra help. We will then come up with a plan unique to your needs, including resources and support to help you live a well-connected, nourished life.
We approach each client with a strengths-based and trauma-informed lens, and treat every person with respect and care. We believe that you are the expert on your own life. You choose the type of support you want, and we will meet you where you are.
